Health Equity and Disparities in Outpatient Services and Referrals by Academic Medical Centers

   Concerns have been raised that academic medical centers refer low-income or uninsured patients to outpatient clinics staffed by residents, while higher-income patients and those with private insurance are referred to faculty practices which contributes to health care disparities and outcomes. 

 

      A June 2008 civil rights complaint by New York Lawyers for the Public Interest, on behalf of Bronx Health REACH, filed with the State Attorney General, alleged that three academic medical centers in New York Citydiscriminated on the basis of payer status and race in their patterns of referral for outpatient services.  There was no action on the complaint.

 

    The Medicaid Redesign Team's Health Disparities Work Group final report issued on October 20, 2011recommended that administrative actions be taken by the New York State Department of Health to assure that the health care delivered by academic medical teaching facilities to persons who are uninsured, to people covered by Medicaid, and to people who are privately insured is comparable and meets the same standards of care.

        

     Legislation on this issue has been introduced which would make separate and unequal care a violation of the Public Health Law.  The current legislation by Assembly Member Nelson Castro (A.7699-A) would require academic medical centers to provide outpatient specialty care in an integrated setting staffed by both attending physicians and residents.  These integrated practices would be required to treat all patients regardless of insurance type or source of payment.

 

    This hearing will review disparities in health care access and outcomes relating to hospital outpatient care. The Committee is interested in comments and recommendations relating to A.7699-A.

 

    Persons wishing to testify at the hearing or planning to attend the hearing should complete and return the enclosed reply form by Monday, May 7.  It is important that the reply form be fully completed and returned so that persons may be notified in the event of emergency postponement or cancellation of the hearing.  Persons who do not testify in person at the hearing are invited to submit written testimony (see below).

 

     All testimony at the hearing will be under oath.  Oral testimony will be limited to 10 minutes' duration.  In preparing the order of witnesses, the Committee will attempt to accommodate individual requests to speak at particular times in view of special circumstances.  These requests should be made on the attached reply form or communicated to Committee staff as early as possible. 

 

      Ten copies of any prepared testimony should be submitted at the hearing registration desk.  The Committee would appreciate advance receipt of prepared statements.  Written testimony, whether presented in person at the hearing or not, should be e-mailed (as a Word or PDF document) before the hearing, or as soon as possible after the hearing, to: Richard Conti at: contir2@assembly.state.ny.us.

7th Annual Dine & Learn Healthy Youth Symposium 

For the past seven years, SUNY Downstate, under the Offices of Program Development and Physician Outreach, has been at the forefront of educating young people and bringing services to them by engaging local partners, tapping local resources and applying for government and philanthropic grants.  Despite our weak economy, schools, providers, public as well as private sectors, and communities understand that supporting healthy young people is a smart investment.  The current program will involve a multidisciplinary team of physicians from SUNY Downstate and other professionals, presenting culturally sensitive health talks on mental health, obesity and diabetes, STDs/HIV/AIDS, and teen dating violence.  Other medical experts will available to offer health screenings and support services.   It is clear that we must take action--together.  With the support of our community partners, we have embarked on this challenge of preventing disease in youth and promoting health during these crucial years.

Stop Fighting Fashion to Live For……Condoms are Fashion For Life!  
For the past four years,our exciting fashion show has been a special feature of the event, generating the curiosity and the attention of young people.   The show promotes condom couture designs not only for the "sheer" purpose of artistic expression, but with the hopes of increasing HIV/AIDS awareness and changing the negative perception surrounding the usage of condoms.  We strongly believe that the fashion and music industries can help re-define the image of condoms by breaking taboos and stereotypes that make teens see the use of condoms as an act of mistrust, promiscuity instead of a gesture of love – the latest in  protection gear.  Through symbolism and popular music (hip hop), the fashion show stresses the importance of delaying sexual activity or engaging in safer sex practices.  Each year we work with fashion designers to create outfits that represent safe sex practices and the importance of delaying sexual activity until both parties are mature to understand the implications involved in sexual activity.  The goal is to make safer sex education fashionable and fun.

In the finale, young people will be exposed to the arts, highlighting youth performances that focus on HIV awareness and other issues that affect them in the community.  The finale will showcase talented students throughout NYC with a special interest on young people living in Brooklyn who have a passion in the areas of art, music and the entertainment industry.  The itinerary will include artists/students from almost any outlet imaginable including: comedy, poetry, spoken word, singing, dancing, stepping, theater, marching arts such as marching bands.
